A. Your firm is invited to submit an offer under seal to Owner for remodel of and addition to an existing building
housing clinic, radiology, lab, pharmacy departments and business offices, located at 211 US-61 South,
Woodville, Mississippi. Bids shall be submitted via email before 2:00 pm local standard time on the 4th day of
November, 2025 and immediately read aloud publicly. No bid shall be accepted or considered after such
scheduled time.

2. At stated time, electronic bids shall be publicly read aloud via Zoom meeting. To receive an invitation to
the Zoom meeting, provide bidder's contact information and email address to Anna Pratt -
apratt@bwbr.com.
B. A pre-bid conference is scheduled for 11:00 am local standard time on the 15th day of October, 2025 at the
proposed project site: 211 US-61 South, Woodville, Mississippi.
C. Questions must be submitted by 5:00 pm local standard time on the 23rd day of October, 2025. The final
addendum will be issued no later than the 28th day of October, 2025.

E. Architect's Project Number: 3.2020181.01.
F. General Contract bidders, upon request, are required to submit Contractor's Qualification Statement.
1. Utilize AIA A305.
2. Provide documentation to indicate a minimum of 7 years experience in healthcare related construction.
G. Plans and specifications can be viewed and purchased online, after September 30, 2025, at Jackson Blueprint

H. Bidders will be required to provide Bid security in the form of a Bid Bond of a sum no less than 5 percent of the
Bid Amount .
I. Refer to other bidding requirements described in Document 00-2113 - Instructions to Bidders .
J. Submit your offer on the Bid Form provided. Bidders may supplement this form as appropriate.
K. Bids are to be received in full compliance with Mississippi Code 1972, Section 31-3-1 et seq and Section 31-7-
13. Awards based on conditions stated in Instructions to Bidders, Specifications, and indicated on Drawings.
L. Bidders must hold a Certificate of Responsibility to comply with Sections 31-3-1 through 31-3- 23 Mississippi
Code of 1972 and amendments thereto and be licensed by the Mississippi Department of Revenue. As per
section 31-3-21, all bids submitted for public projects where said bid is in excess of $50,000 shall contain on the
outside of the envelope the “contractor’s current certificate of responsibility number”. No bid shall be opened
unless such certificate number appears on the envelope or unless there appears a statement on the outside of
the envelope to the effect that bid enclosed therewith did not exceed $50,000. The Certificate of Responsibility
Number or Statement as thus noted; the Project Number and Name (Project # 3.2020181.01 - Field Health
System Catchings Clinic) and the bidder’s name and address must be clearly indicated on the outside of the
sealed bid envelope or said bid will not be opened and/or considered.
M. This agreement is for services related to a project that is subject to the Build America, Buy America Act (BABAA)
requirements under Title IX of the Infrastructure Investment and Jobs Act (“IIJA”), Pub. L. 177-58. Absent an
approved waiver, all iron, steel, manufactured products, and construction materials used in this project must be
produced in the United States, as further outlined by the Office of Management and Budget’s Memorandum
M-22-11, Initial Implementation Guidance on Application of Buy America Preference in Federal Financial
Assistance Programs for Infrastructure, April 18,2022.
N. Your offer will be required to be submitted under a condition of irrevocability for a period of 60 days after
submission.
O. The Owner reserves the right to accept or reject any or all offers.

Purchase of entire printed bid set includes all plans and specifications, including any addendas released at time of order. Planholders will be notified by email when future addendums are issued
and it will be the responsibility of each bid set holder to either download the addenda from the website, or contact Jackson Blueprint to place an order for a printed copy of the addenda.
Shipping, if required, is added to any order based on method selected. Out-of-State orders will not be charged sales tax. In-state orders will be charged 8.0% sales tax.
Online orders are fulfilled within 1-2 business days. Credit card authorization and verification must be received prior to processing. All sales are final.

ADDENDUM DISTRIBUTION
Existing planholders will be notified by email upon the release of any addendum. Failure to order bid sets through Jackson Blueprint and not being on the planholders list may prevent you from
being notified of issued addenda. Printed copies of addendum will be available as part of your bid package upon request by contacting Jackson Blueprint.
Addenda notifications are sent via email from the mail account of “notification@jaxblue.com” and could be flagged as spam by your company network firewall. To prevent not receiving timely notification of these addenda, please contact your company Information Technology Manager and ask that he “white list” the domain of jaxblue.com to prevent emails from being captured.
