4. ARCHITECT'S PROJECT NUMBER: 23-078
5. PROJECT DESCRIPTION: ALL POROUS AND SEMI POROUS BUILDING COMPONENTS THAT MAY HAVE OR WILL
COME
IN CONTACT WITH THE CONTAMINATED BUILDING MATERIALS SHOULD BE REMEDIATED BY
DEMOLITION. THIS
MAY INCLUDE, BUT IS NOT LIMITED TO INTERIOR DRYWALL, STUDS, INSULATION, CEILING
TILES AND GRIDS,
FLOOR TILE, DOORS, FURNISHING, FIXTURES, ETC... THESE ITEMS SHALL BE REMOVED PRIOR
TO ANY WORK TO
DECREASE THE POSSIBILITY OF SPREADING MOLD SPORES ONTO THEM. AT THE
COMPLETION OF THE
REMEDIATION, THE ENTIRE BUILDING SHALL BE FOGGED WITH AN EPA RECOGNIZED
MOLD SPORICIDE/FUNGICIDE.
6. CONTRACT DOCUMENTS MAY BE OBTAINED FOR VIEWING AND PURCHASE AT:

B. The Contractor will be responsible for all costs related to printing and distribution to subcontractors.
7. BIDDERS WILL BE REQUIRED TO PROVIDE BID SECURITY IN THE FORM OF A BID BOND OF A SUM NO LESS
THAN 5 PERCENT OF THE BID AMOUNT.
8. REFER TO OTHER BIDDING REQUIREMENTS DESCRIBED IN DOCUMENT 00.2113 - INSTRUCTIONS TO BIDDERS.
9. SUBMIT YOUR OFFER ON THE BID FORM PROVIDED. BIDDERS MAY SUPPLEMENT THIS FORM AS
APPROPRIATE.
10. YOUR OFFER WILL BE REQUIRED TO BE SUBMITTED UNDER A CONDITION OF IRREVOCABILITY FOR A PERIOD
OF 60 DAYS AFTER SUBMISSION.

Please note that only registered plan holders may bid as a General Contractor.
A printed bid set is $25.00 plus $20.00 shipping and applicable sales tax.
An electronic downloadable set is $25.00 plus applicable sales tax.

Purchase of entire printed bid set includes all plans and specifications, including any addendas released at time of order. Planholders will be notified by email when future addendums are issued and it will be the responsibility of each bid set holder to either download the addenda from the website, or contact Jackson Blueprint to place an order for a printed copy of the addenda. Shipping, if required, is added to any order based on method selected. Out-of-State orders will not be charged sales tax. In-state orders will be charged 8.0% sales tax.
Online orders are fulfilled within 1-2 business days. Credit card authorization and verification must be received prior to processing. All sales are final.
ADDENDUM DISTRIBUTION
Existing planholders will be notified by email upon the release of any addendum. Failure to order bid sets through Jackson Blueprint and not being on the planholders list may prevent you from being notified of issued addenda. Printed copies of addendum will be available as part of your bid package upon request by contacting Jackson Blueprint.
Addenda notifications are sent via email from the mail account of “notification@jaxblue.com” and could be flagged as spam by your company network firewall. To prevent not receiving timely notification of these addenda, please contact your company Information Technology Manager and ask that he “white list” the domain of jaxblue.com to prevent emails from being captured.
